Chronic Disease Management
A workshop for Professionals in Primary Care who wish to make a difference for people with long term conditions.
Why this program?
Rates of chronic disease have been rising.
50% of people aged 65 and over in Australia now have at least 2 long term health conditions
Health professional understanding of the evidence based Chronic Care Model is suboptimal and this program is designed to help the primary care team (nurses, GPs and practice managers) engage with their patients, and increase the value and satisfaction of their work
This program has a focus on delivering care for those that really need it in a General Practice. The emphasis is on planning for wellness versus care planning as a transactional task.
Our approach is about learning together with peers who not only share their expertise, but also their personal support.
The Program
Our approach is about learning together with peers who not only share their expertise, but also their personal support.
Why now?
Post COVID, the curve we need to flatten is the one where many people with sub optimally managed chronic conditions are presenting to crowded hospital emergency departments (EDs) with complications, and flare ups
The experience of presenting late and receiving care in EDs can be changed, and much could be done differently in their primary care practice if the team was better prepared, understood the issues and used the practice data in a proactive manner.
Many health professionals have come from an acute care background where people’s issues are treated and fixed.
Chronic long-term conditions are not “fixed”, but they can be better managed so people can live, work and play with reduced complications, less sick days, and improved quality of life. Health professional understanding of approaches to proactive chronic condition management can be improved to make a difference to the communities they serve.
